To Access Your Eye Care Records
Go to: www.myeyecarerecords.com
1. Register (as 1st time user): Enter registration information (Initial password 1234)
– Then, Enter your new password
2. You will then be redirected to the log in page
– Enter your email and new password
In order to access your records you must provide your social security number at the time of check in.
If you cannot access your records they may not have been *submitted to the portal yet.
*Visits after October 1, 2011 will be submitted, none prior.
